Does deleting the app cancel my Telegram Premium subscription?

No. Removing an app from your phone does not stop the billing. The subscription is held by whoever charges you — the service itself, or the app store you bought it in — and has to be cancelled there.

Cancelled and still charged?

Then the charge is not coming from Telegram Premium. Whoever took your money holds the subscription — if you subscribed inside an app, it is the app store, and only the app store can stop it.
